Driving Directions:

Our Putnam office is located directly on the grounds of the Putnam Hospital Center in Carmel, New York.

Directions from I-84: Get off Exit 19 (Carmel). At end of ramp, turn right onto Route 312 drive a little over a mile to the light. At light, turn right onto Route 6 and drive approximately 1-1/2 miles to third light. Turn left at light onto Stoneleigh Avenue hospital is 2 miles up on the left. When you enter the grounds you can proceed directly to the hospital center.

Directions from I-684: Get off at Exit 8 (Hardscrabble Road/Croton Falls). End of ramp, follow directions to Croton Falls drive a little over 1 mile, going through the town of Croton Falls to sign indicating Brewster straight ahead/Lake Mahopac left. Turn left go .1 miles to light. At light, turn right onto Route 100/202 and go .1 mile. Turn left onto Stoneleigh Avenue go approximately 2-1/2 miles and hospital is on the right. When you enter the grounds proceed directly to the hospital center.

Internal Medicine - Take entrance at valet parking, take elevator to the 2nd floor, make left off of elevator, make your 1st right and Internal Medicine is the last door on the right, Suite 204.

Ob/Gyn – Take entrance at valet parking, take elevator to the 2nd floor, make left off of elevator and Ob/Gyn will be at the end of the hall, Suite 201.

Urology – Take entrance at valet parking, take elevator to the 2nd floor, make left off of elevator, make your 1st right and Urology is the last door on the right, Suite 204.
